Importantly, the investigators say, the protein -- NY-ESO-1 -- is already at the center of a clinical trial underway at the National Cancer Institute. That trial is designed to activate the immune systems of patients with other types of tumors that express the protein, training the body to attack the cancer and eradicate it.

"Typically there is a lag time before a laboratory finding like this leads to a clear path forward to help patients. But in this case, since there is already a clinical trial underway, we have a chance of helping people sooner rather than later," says Gregory J. Riggins, M.D., Ph.D., a professor of neurosurgery at the Johns Hopkins University School of Medicine and the senior author of the study published online in the journal Cancer Immunology Research.

In the NCI trial, NY-ESO-1 is found in a much smaller percentage of tumors than Riggins and his team found in high-grade meningioma, suggesting that for the brain cancer, the target would be potentially more significant.

Most low-grade meningiomas located in easy-to-reach locations can be treated successfully with surgery and radiation. But more atypical, higher-grade tumors are much more difficult to eradicate and are deadlier.

Riggins and his colleagues, including Gilson S. Baia, Ph.D., and Otavia L. Caballero, M.D., Ph.D., set out to find cancer antigens in meningioma. Cancer antigens are proteins expressed in tumors but not in healthy cells, making them good targets for chemical or immune system attack. They looked specifically at 37 cancer/testis (CT) genes, which are not found in normal cells in the body except in germ cells and cells cordoned off in the testicles or, in some cases, ovaries.

CT genes are activated, however, in various cancers. While they are seen as "foreign" by the body's immune system, they are often locked behind the sophisticated defense system that cancers use to evade attack by immune cells. Finding a way to get the immune system to see these protein antigens, however, could allow for the body to recognize the invasion and go after the cancer cells. Various approaches are being used to do that, including vaccines and a system involving removing T-cells from the body and reprogramming them before returning them and setting them loose on the cancer cells.

The Johns Hopkins researchers took tissue from 18 different meningioma samples, removed the genetic material and protein and checked at what levels the 37 different genes were turned on. The gene that is the blueprint for the NY-ESO-1 protein was turned on more frequently than any other, in five of the 18 patient samples.

Then they analyzed NY-ESO-1 expression in a larger group of 110 meningioma tissue samples. They found NY-ESO-1 in 108 of them. The more expression in the sample, they also determined, the higher the tumor grade. The higher levels of NY-ESO-1 expressed also correlated with significantly lower disease-free and overall survival rates in the patients they came from.

The NCI trial originally began in melanoma patients. NY-ESO-1 is expressed in roughly one-third of melanomas as well as approximately one-third of breast, prostate, lung, ovarian, thyroid and bladder cancers, as well as sarcomas. Riggins and his team did not find the protein in glioblastoma, the deadliest form of brain cancer.

He calls the fact that the NCI trial could now include meningioma patients a "stroke of luck."

"If that therapy did not exist, there would be a lot of work that would have to be done to convince people to pursue this," Riggins says. "Our goal is to get something that works to the patients. This puts us well on our way."

"Biliary anatomy misidentification during cholecystectomy [gallbladder removal] can result in injury to the common hepatic duct or common bile duct. Common duct injuries cause significant short- and long-term morbidity including major operations, multiple hospitalizations, and biliary strictures. Elimination of common duct injury is desirable, but it has remained stubbornly present with rates ranging from 0.3 percent to 0.5 percent," according to background information in the article. "When routinely used, intra-operative cholangiography is thought to prevent common duct injury. However, controversy exists regarding the effectiveness of routine use in the prevention of common duct injury."

Kristin M. Sheffield, Ph.D., and Taylor S. Riall, M.D., Ph.D., of the University of Texas Medical Branch, Galveston, and colleagues investigated the association between intraoperative cholangiography use during cholecystectomy and common duct injury, using instrumental variable analysis, an effective way to overcome unmeasured confounding (i.e., factors influencing outcomes not found in the database). The researchers identified Medicare beneficiaries from Texas Medicare claims data who underwent inpatient or outpatient cholecystectomy for conditions including biliary colic or biliary dyskinesia, acute cholecystitis, or chronic cholecystitis. Percentage of intraoperative cholangiography use at the hospital and by surgeon were the instrumental variables. Patients with claims for common duct repair operations within 1 year of cholecystectomy were considered as having major common duct injury.

A total of 92,932 Medicare beneficiaries 66 years or older underwent cholecystectomy at 307 hospitals in Texas from 2001 through 2009. There were 37,533 cholecystectomies with intraoperative cholangiography (40.4 percent) and 55,399 without (59.6 percent). Common duct injury occurred in 280 patients (0.30 percent). There were 201 common duct injuries (0.36 percent) in patients undergoing cholecystectomy without intraoperative cholangiography and 79 injuries (0.21 percent) for those having an intraoperative cholangiography.

"In a logistic regression model controlling for patient, surgeon, and hospital characteristics, the odds of common duct injury for cholecystectomies performed without intraoperative cholangiography were increased compared with those performed with it. When confounding was controlled with instrumental variable analysis, the association between cholecystectomy performed without intraoperative cholangiography and duct injury was no longer significant," the authors write.

"Significant controversy exists regarding the role of intraoperative cholangiography in the prevention of common duct injury during cholecystectomy. Previous population-based studies using data from Medicare claims, hospital discharge records, and national inpatient registries report nearly 2-fold higher rates of injury in cholecystectomies performed without intraoperative cholangiography. In the present study using Texas Medicare claims data, the association between intraoperative cholangiography and common duct injury was highly sensitive to the analytic method used."

"Failure to account for potentially confounding variables not routinely captured in administrative databases has a major effect on the interpretation of the findings. Intraoperative cholangiography was not associated with significant reduction in common duct injury using instrumental variable analysis. Instrumental variable analysis balances unmeasured confounding variables to better align risk factors in comparator groups. With better control for unmeasured confounding variables, intraoperative cholangiography was no longer associated with common duct injury. Based on these results, routine intraoperative cholangiography should not be advocated as means for preventing common duct injury," the researchers conclude.

Inflammation Producing Molecular Pathway Is Cause Of Pulmonary Hypertension: Researchers

The results, which will be published Aug. 28 in Science Translational Medicine, suggest that using medications to block this pathway could lead to the first-known cure for the disease, apart from lung transplantation. The new research could also lead to a better understanding of other diseases involving inflammation of blood vessels, such as coronary artery disease, said Mark Nicolls, MD, senior author of the study and division chief of pulmonary and critical care medicine at Stanford, as well as a staff physician at the Veterans Affairs Palo Alto Health Care System.

"We believe that targeting inflammation is an exciting approach to augment current treatments for pulmonary hypertension because it may reverse the underlying cause of the disease," said Nicolls, who is also director of the Lung Immunology Program and an associate professor of medicine. "We believe this is going to be an approach that helps a large number of patients."

The lead author of the study is Wen Tian, PhD, a research associate in Nicolls' lab.

Pulmonary hypertension, while rare, usually strikes young and middle-aged women, leaving them short of breath and often unable to complete simple daily tasks. The condition can be fatal. The risk is higher for people with certain autoimmune diseases (such as scleroderma or lupus), HIV, congenital heart disease or liver disease. The risk also is higher for those who have used street drugs, such as amphetamines or cocaine, or the diet drug fenphen.

About 100,000 people in the United States and Europe have been diagnosed with pulmonary hypertension, but many others are believed to go undiagnosed because the main symptom, shortness of breath, is nonspecific. Until the 1990s, there were no treatments except lung transplantation, which has varying degrees of success.

What is known about the disease is that the narrowing of blood vessels in the lungs is caused by a mysterious proliferation of the smooth muscle cells that ring those vessels. As the vessel walls thicken, they become increasingly occluded, choking off blood flow.

Blood vessels are elastic tubes that have an outer layer of smooth muscle and an inner layer of endothelial cells. The current treatment for patients with pulmonary hypertension is vasodilators, drugs that cause smooth muscle cells to relax, permitting more blood to flow through the diseased vessels. These drugs help to extend survival and relieve some symptoms, but they don't provide a cure.

"Current approved therapies for pulmonary hypertension have focused on dilating blood vessels without special attention being paid to the inflammation that is frequently seen around blood vessels," Nicolls said. "We were interested in finding out how inflammation contributes to the disease."

In laboratory experiments, researchers removed inflammation-producing cells called macrophages from the lung tissue of rats dying of pulmonary hypertension and put the cells in cultures with healthy rat endothelial cells.

"We were shocked to find that half of the endothelial cells were dead within 24 hours," Nicolls said. "That totally surprised us."

In subsequent lab experiments, researchers found that if they blocked the molecular pathway triggered by the macrophages, the endothelial cells didn't die. Researchers conducted similar experiments in human tissue taken from the diseased lungs of patients. The results were similar. "The same pathways were at play in the human lungs," Nicolls said.

In experiments in live rats with induced pulmonary hypertension, the disease was reversed when researchers blocked the inflammation-producing pathway.

"It was dramatic," Nicolls said. "We could start the drug as late as three weeks when animals were breathing hard and walking slowly, and the disease reversed."

Mosquitoes Smell You Better at Night: Study

The study reports an integrative approach to examine the mosquito's ability to smell across the 24-hour day and involved proteomic, sensory physiological, and behavioral techniques. The researchers examined the role for a major chemosensory family of mosquito proteins, odorant-binding proteins (OBPs), in the daily regulation of olfactory sensitivities in the malarial mosquito. It is thought that OBPs in the insect antennae and mouth parts function to concentrate odorant molecules and assist in their transport to the actual olfactory receptors, thereby allowing for odorant detection. The team revealed daily rhythmic protein abundance of OBPs, having higher concentrations in the mosquito's sensory organs at night than during the day. This discovery could change the way we look at protecting ourselves from these disease-carrying pests.

The team also included Matthew M. Champion, Eck Institute for Global Health Research Assistant Professor in the Department of Chemistry and Biochemistry, who specializes in proteomics.

This study utilized mass spectrometry to quantify protein abundance in mosquito sensory organs, and electroantennograms to determine the response induced by host odorants at different times of the day. The coincident times of peak protein abundance, olfactory sensitivity and biting behavior reflect the extraordinarily fine-tuned control of mosquito physiology. Olfactory protein abundance and olfactory sensitivity are high when needed (at night) and low when not required (daytime).

Samuel Rund, a doctoral candidate in the laboratory of Duffield and a former Eck Institute for Global Health Fellow, and Nicolle Bonar, a visiting undergraduate student from Queens University of Ontario, Canada, were the lead authors on this research. The Notre Dame team also included then-undergraduate student John Ghazi, Class of 2012; undergraduate Cameron Houk, Class of '14; and graduate student Matthew Leming.

Rund noted, "This was an exciting opportunity to bring many people and techniques together to make some really fascinating findings on the mosquito's ability to smell humans, its host. Just think, during the day the mosquito is sleeping and doesn't need to smell you. But when the sun goes down, the mosquito's olfactory system becomes extra-sensitive, and she is ready to smell and then bite you."

The project was a follow-up to their earlier work that utilized genomic tools to reveal 24-hour rhythmic patterns of gene expression, including many genes involved in olfaction.

Rund and Duffield's earlier work with collaborator James Gentile from Notre Dame's Department of Computer Science and Engineering, "Extensive circadian and light regulation of the transcriptome in the malaria mosquito Anopheles gambiae," helped lay some of the foundation to their findings. The paper, published in BMC Genomics in April, further examined the regulation of rhythms in gene expression at the molecular level, highlighted important differences in biological timing between Anopheles gambiae and the important dengue vector, Aedes aegypti, and highlighted the important role of light in organizing and modifying gene expression.

Anopheles gambiae is the primary species that is responsible for the transmission of malaria in sub-Saharan Africa, with approximately 300 million infections and 1 million deaths annually. The fact that these studies were conducted in Anopheles gambiae mosquitoes has important implications for the development of novel insect control methods with the potential to reduce the transmission of malaria parasites and thus the morbidity and mortality associated with malaria disease. This work provides the first comprehensive evidence of the important role of daily rhythms in the sensory biology of Anopheles gambiae and the implications for developing new control methods.
